Job summary

Telecare Corporation is seeking a dedicated on-site crisis services case manager to provide screening, assessments and connect clients with needed services. Under supervision, you will support members and families to navigate crisis episodes and recovery paths.

The role requires 18+ with direct service experience in crisis intervention, and certification by start date. The Windsor Center offers 24/7 crisis stabilization in a supportive environment.

Qualifications

• High School Graduation or GED with 3 years direct mental health/community services experience; or an Associate's degree with 2 years; or Bachelor's in Social Services.
• At least 1 year of direct service experience with Crisis Intervention, including screening and assessments.
• Sensitivity to multi-cultural populations and issues; must be 18 years or older.
• CPR, CPI and First Aid certification required by start or within 60 days and maintained.

Responsibilities

• Provide screening, assessment and assist in linking services to members and families to overcome crises.
• Serve as primary support and case manager under supervision to coordinate care.
• Collaborate with care teams to ensure timely access to crisis services and community resources.
