Job summary

Easterseals NH in Manchester, NH seeks a Community Living Manager to provide case management for adults in foster care homes, coordinating with providers, guardians and healthcare staff. You will recruit and train home providers and respite staff, and develop treatment plans with consistent implementation across programs.

The role requires a Bachelor’s degree or equivalent experience, with supervisory background preferred.

Qualifications

• Bachelor’s degree in rehabilitation, human services or related field.
• Six years of relevant experience may substitute for the educational requirement.
• One year supervisory experience is preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide case management for adult foster care homes and individuals with developmental disabilities.
• Recruit, train and monitor Home Providers and respite providers.
• Maintain communications with clients, program staff, case managers, guardians, parents, physicians, etc.
• Develop and ensure implementation of treatment plans.
