Job summary

Easterseals NH in Manchester, NH is hiring a Family Outreach Worker for our Residential Educational Services program. You will provide case management to youth in our residential program and to families in the greater Manchester area.

Qualifications

• Bachelor's Degree in Human Services or related field.
• Two years of experience working with children and families.
• Valid driver's license, reliable transportation, Auto Insurance.

Responsibilities

• Work with social services to provide family-centered practices, including work in the community.
• Assess the family's community resources and assist the youth and family in identifying strategies to support the youth and family.
• Work with the residential and clinical team to support clients and help them work to achieve their treatment goals.
• Ensure all medical appointments are up to date.
• Be responsible for all communication with families and collaterals.
• Keep compliant with documentation including authorizations and reporting.
