Job summary

Heart to Heart Hospice seeks an On-Call Registered Nurse to perform home visits for patient assessment and care. You will provide supportive nursing care, crisis management, and coordination with physicians to develop individualized care plans for patients and families.

The role requires an active RN license, safe driving, and at least 2 years of clinical experience. You will join a compassionate team dedicated to high-quality end-of-life care and community service.

Qualifications

Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
Current State Registered Nurse License.
Current Driver's License with reliable transportation.

Responsibilities

Provide home visits for patient assessment, treatment, and support, including crisis management and pronouncement when appropriate.
Develop and revise nursing care plans in collaboration with the physician and patient/family.
Maintain accurate clinical notes and communicate changes in patient needs to the physician and care team.
Ensure timely documentation and compliance with Medicare regulations and internal policies.
