Job summary

Heart to Heart Hospice is seeking a Registered Nurse On Call to provide compassionate, at-home patient care. The role includes home visits for assessment and treatment, planning and revising care plans with physicians, and maintaining accurate documentation while coordinating with the care team.

Required are a nursing degree and active Texas RN licensure; two years of RN experience is preferred. A valid driver's license and reliable transportation accompany the duties, including potential use of

Qualifications

• RN license current in state of Texas.
• Two years of RN experience in healthcare/clinical setting preferred.

Responsibilities

• Provide home visits for patient assessment, treatment and support.
• Develop and revise nursing care plans in collaboration with physician and IDG.
• Maintain professional communication with patients, families and healthcare team.
• Prepare progress notes showing goals and outcomes.
• Inform changes in patient needs to physician and team; monitor response to care.
• Submit all documentation timely and accurately.
• Perform other duties as required and ensure compliance with laws and policies.
