Job summary

CVS Health Corporation is seeking a Case Management Coordinator for a full-time field-based telework role in Ohio. The position requires travel within Western Ohio up to 50% and a schedule of Monday–Friday, 8–5 with flexibility for after-hours needs.

The role supports OhioRISE, coordinating care for children with complex behavioral health needs, collaborating with caregivers, MCOs, and providers to ensure timely and appropriate services.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers, keyboard and MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or a public sector human services or behavioral health care field.

Responsibilities

• Collaborate and inform the case management process to facilitate appropriate healthcare outcomes.
• Evaluate members’ needs/eligibility and recommend an approach to case resolution.
• Coordinate and implement care plan activities and monitor progress.
• Identify and escalate quality of care issues and collaborate with teams to overcome barriers.
