Job summary

Evolve Therapy Services seeks a Certified Occupational Therapy Assistant to provide in-house therapy services at an on-site Skilled Nursing Facility in Church Hill, TN. This PRN position requires an active OTA license and an associate degree.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Screen residents to determine best intervention.
• Implement treatment plans under supervising therapist.
• Communicate findings to residents, families/caregivers, and team.
• Establish resident therapy schedules with team members.
• Document and bill occupational therapy services per plan.
• Comply with CMS documentation standards daily.
