Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to provide in-house therapy services at our partner facility in Church Hill, Tennessee. This PRN role focuses on implementing established plans of care and supporting residents' functional goals.

You will collaborate with therapists and the interdisciplinary team, conduct screenings as needed, document per CMS standards, and help ensure high-quality resident care.

Qualifications

• Associate's degree in Occupational Therapy
• Active OTA licensure required; ongoing continuing education to maintain licensure and certification

Responsibilities

• Conduct regular resident screenings to determine intervention or treatment
• Perform treatment plans following established goals and care plan
• Interpret and communicate evaluation findings to residents, families, and interdisciplinary team
• Establish resident therapy schedules with team members
• Conduct therapy per treatment plan and ensure documentation and billing compliant with CMS standards
• Perform other duties as needed to effectively treat patients

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
