Evolve Therapy Services seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Newport, TN. This position supports residents with personalized therapy plans and works under an supervising therapist to ensure quality care.

Ideal candidates hold an OTA license, possess an associate degree in occupational therapy, and commit to ongoing professional development and CMS documentation standards while collaborating with an interdisciplinary team.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsabilités

• Screen residents to determine best intervention plan.
• Implement treatment plans and follow the established plan of care with supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and families.
• Establish individual resident therapy schedules with the care team.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point of care documentation.
