Job summary

Evolve Therapy Services in Jellico, TN, is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. This PRN position supports resident care and collaborates with a multidisciplinary team.

The role emphasizes screening, implementing treatment plans, documentation, and billing, with growth opportunities within the organization. Must hold OTA licensure and an Associate's degree in Occupational Therapy.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education
