The Grand Rehabilitation and Nursing at Barnwell is seeking a licensed Occupational Therapist to evaluate, develop, and implement individualized treatment plans that help residents improve functional independence and quality of life. This role emphasizes patient-centered care, interdisciplinary collaboration, and work-life balance.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current New York State OT license.
• Experience in skilled nursing or long-term care preferred (not required).
• Strong clinical judgment, communication, and documentation skills.
• Ability to work independently and as part of a collaborative rehab team.

Responsabilités

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Implement treatment plans focused on improving functional independence and daily living skills.
• Assess and document resident progress through daily notes, progress reports, and re-certifications.
• Maintain accurate and compliant clinical documentation.
• Ensure confidentiality of resident information at all times.
• Supervise Occupational Therapy Assistants (COTAs) as assigned.
• Participate in care plan meetings and interdisciplinary collaboration.
• Assist with departmental projects, reporting, and data tracking as directed by the Rehab Director.

About The Role

Occupational Therapist Key Responsibilities
