Job summary

(Colonial Vista Post Acute) Colonial Vista SNF Healthcare, LLC is seeking a licensed Occupational Therapist for a full-time role at our United States facility. The OT evaluates residents, develops care plans, and coordinates with physicians and families to support mobility and independence.

You will supervise COTA staff, train aides, participate in care conferences, and ensure compliant documentation. A Bachelor’s degree in OT and a state license are required; CPR may be needed per licensure.

Qualifications

• Bachelor’s degree in occupational therapy; state license required.
• Proficient computer skills for Net Health and Point Click Care.
• Licensed OT in good standing; CPR as required by licensure.

Responsibilities

• Evaluate residents to determine OT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise COTA and aides; train staff and participate in care conferences.
• Maintain timely, accurate documentation per policies and payer requirements.
• Educate residents and families on treatment goals and home programs.
