Job summary

Tice Valley Post Acute is seeking a compassionate Occupational Therapist to join our rehabilitation team on a full-time basis. You will evaluate residents, create individualized plans, and deliver services to improve daily living and independence.

Located in a resident-centered setting, you will collaborate with physicians, nurses, CNAs, and other therapists to support recovery and safety, documenting outcomes and adjusting plans as needed for optimal resident care.

Qualifications

• CA Occupational Therapist license required.
• Graduate of an accredited Occupational Therapy program.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.

Responsibilities

• Evaluate residents' functional abilities and develop individualized treatment plans.
• Provide OT services to improve strength, mobility, coordination, and independence in ADLs.
• Implement therapeutic interventions based on residents' needs and care plans.
• Assist residents with activities of daily living (ADLs).
• Assess and address safety, mobility, and environmental needs.
• Monitor progress and modify treatment plans as appropriate.
• Document evaluations, treatments, progress, and outcomes clearly and timely.
• Collaborate with physicians, nurses, CNAs, PTs, SLPs, and the inter-disciplinary team.
• Educate residents and caregivers on techniques and strategies to support independence.
• Recommend adaptive equipment and assistive devices when appropriate.
