Job summary

Park Meadows Post Acute seeks a Staff Occupational Therapist to evaluate residents, create individualized treatment plans, and oversee COTA and aides. You will collaborate with physicians and the care team to support mobility, balance, and independence.

The role emphasizes ongoing staff training, documentation, and patient/family education on home programs and safety. CPR and state licensure are required as applicable.

Qualifications

• Bachelor’s degree in Occupational Therapy is required.
• Licensure as an Occupational Therapist in the state.
• CPR certification if required by licensure.

Responsibilities

• Evaluate residents to determine OT needs and rehabilitation potential.
• Develop and implement individualized treatment plans to improve mobility and independence.
• Supervise COTA, aides, and students in resident care activities.
• Maintain accurate documentation per facility and payer requirements.
• Educate residents and families on goals, home programs, and safety techniques.
