Job summary

PACS is seeking a Staff Occupational Therapist in Portland, OR to assess residents, design individualized treatment plans, and collaborate with a multidisciplinary team. The role includes supervising OT assistants, educating families, and ensuring accurate documentation and compliance with regulations.

The OT will work closely with physicians, nursing staff, and rehabilitation teams to optimize mobility and independence, while maintaining safety and quality in care delivery.

Qualifications

• Bachelor's degree in Occupational Therapy required; Masters/Doctorate preferred.
• Licensed to practice Occupational Therapy in the state.
• Strong communication and documentation skills.

Responsibilities

• Evaluate residents to determine OT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise OT assistants and train staff as needed.
• Communicate progress with physicians, nursing staff, and families and participate in care conferences.
• Complete compliant documentation in electronic medical records and adhere to safety protocols.
