Job summary

Continuum Therapy Partners in Willmar, MN is seeking an Occupational Therapist to join our team on a PRN, on-site basis. You will evaluate patients, develop individualized treatment plans, and collaborate with families to promote safety and independence in daily activities.

The role requires a Master's or Doctorate in Occupational Therapy, an active state license, and NBCOT certification. Prior OT experience is preferred, with focus on enhancing quality of life for patients in skilled care

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required
