Job summary

Continuum Therapy Partners in Melrose, MN is seeking an Occupational Therapist to join our PRN team. The role focuses on evaluating patients, developing personalized treatment plans, and supporting functional independence in skilled care settings.

The clinician will collaborate with families and care teams to educate on adaptive equipment and home safety, document progress, and adjust plans based on outcomes. Minnesota licensure and NBCOT are required or in process.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy is required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required
