Continuum Therapy Partners, LLC is seeking an Occupational Therapist for the Cura of Sauk Centre location in Sauk Centre, MN. You will evaluate patients, develop and adjust treatment plans, and work closely with families to promote functional independence in a skilled care setting.

The role emphasizes patient education on adaptive equipment and home safety, with a PRN schedule to meet patient needs. A current OT license and NBCOT certification are required, with experience preferred.

Qualifications

• Must have Master’s or Doctorate in Occupational Therapy.
• Current, unencumbered state OT license; NBCOT certification mandatory.
• 1 year of OT experience preferred.

Responsabilités

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients’ functional independence
