Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist to support evaluation and treatment planning for patients in Willmar, MN. This PRN role emphasizes improving functional independence within a therapist-led network.

Required are a Master’s or Doctorate in OT, an active state license, NBCOT certification, and about one year of prior OT experience. Travel within Cura of Willmar facilities may be involved.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
