Continuum Therapy Partners, LLC is seeking an Occupational Therapist to join our Monticello, MN team on a PRN schedule. We value therapists who listen and advocate for patient-centered care, contributing to a growing network focused on improving quality of life in skilled care settings.

Ideal candidates hold a Master’s or Doctorate in OT with active licensure and NBCOT certification, plus 1 year of experience.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
