Job summary

Continuum Therapy Partners is seeking an Occupational Therapist to join our clinician-led network. The role focuses on evaluating and treating patients, developing individualized plans to improve functional independence, and educating families on equipment and home safety.

Candidates must hold a Master’s or Doctorate in Occupational Therapy, hold a current state OT license, and have NBCOT certification. Prior pediatric or rehabilitation experience is a plus; PRN schedule at Little Falls, MN

• Master’s or Doctorate Degree in Occupational Therapy is required.
• Current, unencumbered state OT license; NBCOT certification is required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities
