Job summary

Innovate Rehab and Wellness in Louisiana is seeking a licensed Occupational Therapist for PRN work in a Skilled Nursing Facility. The role offers flexible, as-needed shifts and a competitive hourly rate based on experience, with a focus on one-on-one patient care.

You will evaluate patients, develop treatment plans, and collaborate with a multidisciplinary team to improve movement, independence and safety. New grads are welcome; CPR certification preferred; valid state OT license required.

Qualifications

Responsibilities

• Conduct thorough OT evaluations.
• Develop and adjust personalized treatment plans.
• Use therapeutic techniques and adaptive equipment.
• Train patients and caregivers on exercises and assistive devices.
• Educate on strategies to improve function and safety.
• Collaborate with PT, SLP, physicians, and the full care team.
• Document patient progress and treatment changes accurately.
• Stay current with OT best practices and research.
