Job summary

Innovate Rehab and Wellness in Ringgold, LA is seeking a PRN Physical Therapist to join our skilled nursing facility team. You will evaluate patients, develop personalized plans, provide hands-on therapy, and collaborate with an interdisciplinary team to restore movement and quality of life.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Valid state PT license.
• CPR certification preferred.

Responsibilities

• Evaluate patients and identify movement and functional limitations.
• Create and lead individualized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
