Innovate Rehab and Wellness is seeking a Licensed Occupational Therapist for a full-time role in a Skilled Nursing Facility. You will evaluate patients, create and deliver treatment plans, and help patients regain independence. The position offers a competitive hourly rate based on experience and a supportive, patient-centered care team.

Qualifications

• Master’s or Doctorate in Occupational Therapy (accredited program) is required or preferred.
• Valid state OT license is required.
• Experience as an OT is preferred (new grads welcome).

Responsibilities

• Conduct thorough OT evaluations.
• Develop and adjust personalized treatment plans.
• Train patients and caregivers on exercises and assistive devices.
• Educate on strategies to improve function and safety.
• Collaborate with PT, SLP, physicians, and the care team.
• Document patient progress and treatment changes accurately.
