Job summary

INNOVATE REHAB AND WELLNESS is seeking an Occupational Therapist in Natchitoches, LA for flexible, PRN shifts in a skilled nursing setting. You will evaluate patients, create and deliver individualized treatment plans, and help improve motor skills and daily function.

The role emphasizes patient-centered care, collaboration with the care team, and ongoing professional development. A state license and CPR may be preferred. This position offers supportive, therapist-owned company culture.

Qualifications

• Master's or Doctorate in Occupational Therapy (accredited program).
• Valid state OT license.
• Experience as an OT preferred (new grads welcome).
• Strong communication and problem-solving skills.
• CPR certification preferred.

Responsibilities

• Conduct thorough OT evaluations.
• Develop and adjust personalized treatment plans.
• Use therapeutic techniques and adaptive equipment.
• Train patients and caregivers on exercises and assistive devices.
• Educate on strategies to improve function and safety.
• Collaborate with PT, SLP, physicians, and the full care team.
• Document patient progress and treatment changes accurately.
• Stay current with OT best practices and research.

Pay: $48.00 - $65.00 based on experience, location and availability

Position Summary
