Job summary

Innovate Rehab & Wellness seeks a licensed Occupational Therapist to join Crestwood Health Care Center in Florissant, MO on a PRN basis. You will evaluate patients, create individualized treatment plans, and help improve daily living activities through patient-centered care.

This role offers flexible scheduling, collaboration with an interdisciplinary team, and opportunities for professional development in a supportive, clinician-owned culture.

Qualifications

• Master’s or Doctorate in Occupational Therapy from an accredited program.
• Valid state OT license is required.
• CPR certification preferred.

Responsibilities

• Conduct thorough OT evaluations.
• Develop and adjust personalized treatment plans.
• Use therapeutic techniques and adaptive equipment.
• Train patients and caregivers on exercises and assistive devices.
• Educate on strategies to improve function and safety.
• Collaborate with PT, SLP, physicians, and the full care team.
• Document patient progress and treatment changes accurately.
• Stay current with OT best practices and research.

Education

Master’s or Doctorate in Occupational Therapy

Valid state OT license

CPR certification preferred

Position Summary
