Job summary

Santa Rosa Post Acute is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team in Santa Rosa, CA. This PRN position offers a $5,000 sign-on bonus and the chance to help residents regain independence through individualized therapy plans.

Evaluate residents, develop treatment plans, and provide skilled OT interventions while collaborating with a multidisciplinary team. You’ll document thoroughly, participate in care conferences, and educate families on home programs

Qualifications

• Current California Occupational Therapist license in good standing.
• Graduate of an accredited Occupational Therapy program.
• Experience in skilled nursing, post-acute, rehabilitation, or geriatric care preferred.
• New graduates with strong clinical rotations encouraged to apply.
• Experience with EMR systems Casamba, Net Health, PointClickCare is a plus.
• Strong communication, organizational, and teamwork skills.
• Passion for helping patients achieve independence and function.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy interventions per physician orders and care plans.
• Help patients improve ADLs, self-care, mobility, cognition, and functional independence.
• Collaborate with Physical, Speech Therapists, nursing staff, physicians, and families to coordinate care.
• Complete timely and accurate documentation in accordance with regulatory and facility requirements.
• Participate in interdisciplinary team meetings, care conferences, and discharge planning.
• Educate patients, families, and caregivers on therapeutic techniques and home programs.
• Maintain compliance with federal, state, and professional standards of practice.

$80-$87/hr. | PRN

Sign‑On Bonus: $5,000

PRN new hires are eligible for a $5,000 sign‑on bonus, paid in two installments:
(50%) after completion of 45 days of continuous employment
(50%) after completion of 90 days of continuous employment

Bonus payments are contingent upon remaining actively employed and in good standing at the time each payment is due.
