Job summary

Marin Post Acute in San Rafael, CA is seeking a compassionate Occupational Therapist (OT) to join our growing rehabilitation team. We provide post-acute and skilled nursing care to help residents regain independence and improve daily function.

You will evaluate residents, develop individualized treatment plans, implement interventions to enhance mobility, strength, coordination, cognition, and ADLs, and educate families while collaborating with physicians, nursing staff, and other therapists.

Qualifications

  • California OT license required.
  • Graduate of an accredited OT program.
  • Strong clinical assessment and treatment skills.
  • Excellent communication, documentation, and interpersonal skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Preferred experience in SNF, post-acute care, rehabilitation, or long-term care.
  • New Graduates are encouraged to apply.

Responsibilities

  • Perform occupational therapy evaluations and assessments.
  • Develop individualized treatment plans based on resident goals and clinical needs.
  • Implement therapeutic interventions to improve mobility, strength, coordination, cognition, and ADLs.
  • Assist residents in regaining independence and maximizing functional abilities.
  • Educate residents, families, and caregivers on treatment plans and adaptive techniques.
  • Recommend and train residents on adaptive equipment and environmental modifications.
  • Collaborate with physicians, nursing staff, therapy team members, and interdisciplinary care teams.
  • Maintain timely and accurate clinical documentation in compliance with facility, state, and federal requirements.
  • Monitor resident progress and modify treatment plans as appropriate.
  • Participate in care conferences and discharge planning.

Occupational Therapist Responsibilities

  • Active and unrestricted California Occupational Therapist (OT) License

  • Ability to work independently and collaboratively within a multidisciplinary team
  • Preferred Experience in Skilled Nursing Facilities (SNF), Post-Acute Care, Rehabilitation, or Long-Term Care
  • Knowledge of Medicare, PDPM, and rehabilitation documentation practices
