Job summary

Blue Oak Post-Acute in Santa Rosa, CA is seeking a licensed Occupational Therapist (OT) to design and implement individualized therapy plans for residents to maximize independence and safety.   • Knowledge of PDPM, Medicare, and SNF regulations.
  • Strong communication, organizational, and teamwork skills.
  • Commitment to resident-centered, high-quality care.

Responsibilities

  • Evaluate residents and develop individualized occupational therapy treatment plans.
  • Implement evidence-based interventions to support functional independence.
  • Document resident progress accurately and timely in accordance with regulatory requirements.
  • Collaborate with interdisciplinary teams to optimize resident care.
  • Participate in care conferences and family meetings, as applicable.
  • Mentor Occupational Therapy Assistants and support staff, as applicable.
