Job summary

Sonoma Post Acute in Sonoma, CA seeks a dedicated Occupational Therapist to join our resident-centered team. You will evaluate residents, develop individualized treatment plans, and collaborate with physical and speech therapists, nursing, and family members to promote independence.

You must hold a California OT license, have experience in rehabilitation or skilled nursing, and demonstrate strong clinical and communication skills to support high-quality care and positive outcomes for residents.

• Current California license as an Occupational Therapist.
• Experience in a skilled nursing or rehabilitation setting preferred.
• Strong clinical skills and the ability to develop and implement effective treatment plans.

Responsibilities

• Evaluate residents' physical and functional abilities, and develop individualized treatment plans.
• Implement therapeutic interventions to improve residents' ability to perform daily activities and achieve their rehabilitation goals.
• Collaborate with an interdisciplinary team, including physical therapists, speech therapists, and nursing staff, to provide comprehensive care.
• Educate residents and their families on techniques to enhance functional independence and ensure a safe environment at home.
• Maintain accurate and timely documentation of assessments, treatment plans, and progress notes.
