Job summary

Santa Rosa Post Acute, part of the PACS network, seeks a compassionate Occupational Therapist (OT) to join our rehabilitation team in Santa Rosa. The role focuses on helping residents regain independence through individualized treatment plans and functional improvements.

Full‑time or part‑time positions are available. Sign‑on bonus of $2,000 is offered, with payments at 90 and 180 days, contingent on continued employment and good standing.

Qualifications

• Current California OT license in good standing.
• Graduate of an accredited Occupational Therapy program.
• Experience in skilled nursing/post-acute/geriatric rehab preferred.
• New graduates encouraged to apply.
• Experience with EMR systems Casamba, Net Health, or PointClickCare is a plus.
• Strong communication, organizational, and teamwork skills.
• Passion for helping patients achieve independence.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Provide skilled OT interventions per physician orders and care plans.
• Improve ADLs, self-care, mobility, cognition, and functional independence.
• Collaborate with PTs, STs, nursing, physicians, and families.
• Complete timely documentation complying with regulations.
• Participate in interdisciplinary meetings and discharge planning.
• Educate patients and families on therapy techniques and home programs.
• Maintain compliance with standards of practice.

Santa Rosa Post Acute is seeking a compassionate, motivated, and patient-centered Occupational Therapist (OT) to join our growing rehabilitation team. Sign‑On Bonus

Full‑time new hires are eligible for a 2,000 dollar sign‑on bonus, paid in two installments: 1,000 (50%) after completion of 90 days of continuous employment; 1,000 (50%) after completion of 180 days of continuous employment. Bonus payments are contingent upon remaining actively employed and in good standing at the time each payment is due.

About Santa Rosa Post Acute

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans
• Provide skilled occupational therapy interventions in accordance with physician orders and established care plans
• Help patients improve performance in activities of daily living (ADLs), self‑care tasks, mobility, cognition, and functional independence
• Collaborate with Physical Therapists, Speech Therapists, nursing staff, physicians, and families to coordinate care
• Complete timely and accurate documentation in compliance with regulatory and facility requirements
• Participate in interdisciplinary team meetings, care conferences, and discharge planning
• Educate patients, families, and caregivers regarding therapeutic techniques and home programs
• Maintain compliance with all federal, state, and professional standards of practice

Qualifications

• Current California Occupational Therapist (OT) license in good standing
• Graduate of an accredited Occupational Therapy program
• Skilled nursing, post‑acute, rehabilitation, or geriatric experience preferred
• New graduates with strong clinical rotations are encouraged to apply
• Experience with Casamba, Net Health, PointClickCare, or similar EMR systems is a plus
• Strong communication, organizational, and teamwork skills
• Passion for helping patients achieve their highest level of function and independence
