Qualifications

Responsibilities

• Evaluate patient needs and develop individualized treatment plans.
• Support patients in regaining physical and cognitive function.
• Guide activities of daily living and therapeutic exercises.
• Adapt treatment based on progress and goals.
• Collaborate with physicians and interdisciplinary teams.
• Educate patients and families on plans and home care.
• Maintain accurate and timely documentation.
• Support discharge planning and continuity of care.
