Job summary

Olympia Transitional Care in Olympia, WA is seeking a full-time Occupational Therapist for our in-house rehab program. You will screen and treat patients with cognitive and ADL impairments, develop plans of care, and collaborate with the health team to ensure quality outcomes.

We welcome new grads and experienced therapists. Our facility offers a supportive environment with a focus on recovery and patient-centered care.

Qualifications

Occupational Therapist license is required.
New grads welcome; all experience levels considered.

Responsibilities

Screen/evaluate patients with cognitive and/or ADL impairments and develop plan of care.
Use standardized assessments and evidence-based practice to support interventions.
Provide rehabilitative and medically necessary treatment per regulatory requirements.
Perform ADL retraining, therapeutic activities, exercises, modalities, and use of assistive devices.
Assess progress and adjust treatments in clinical records.
Evaluate outcomes and modify services based on referrals and results.
Offer counseling to patients and families; communicate with health team about progress and plans.
Provide discharge summaries per regulatory requirements.
