United Health Centers is seeking an Occupational Therapist to screen and assess member functional status within the PACE program. You will plan and implement interventions, communicate with the interdisciplinary team, and provide guidance to Patient Care Aides and Rehab Assistants.

You will also contribute to staff education, conduct group education for members, and participate in care planning and discharge planning as needed.

Qualifications

• Must be licensed to practice OT in California.
• Experience with PACE program and EMR preferred.
• Bilingual English/Spanish preferred.

Responsabilités

• Screen and assess member functional status and plan interventions.
• Provide guidance and supervision to rehabilitation staff and aides.
• Deliver group education to members and participate in care planning.
