Job summary

United Health Centers is seeking a Part Time Occupational Therapist to join our team. The role involves screening and assessing member functional status, planning interventions, and communicating with the interdisciplinary team to maintain functional status, especially for high fall risk members.

The position entails providing guidance to Care Aides and Rehab staff, participating in in-services, and conducting group education for members.

Qualifications

• Bachelor of Science degree in Occupational Therapy from an accredited program.
• Current state licensure in California.
• Experience with PACE program and EMR systems preferred.

Responsibilities

• Screens and assesses member functional status and plans interventions.
• Collaborates with interdisciplinary team for care planning and resource allocation.
• Provides guidance to Rehab staff and conducts staff training as needed.
• Develops and supervises follow-through programs for OT and supports discharge planning.
