Qualifications

• Bachelor's degree in Occupational Therapy from an accredited program.
• Current, active OT license in the applicable state.
• Minimum 1 year of experience with frail/elderly populations; 2 years preferred.

Responsibilities

• Conduct occupational therapy assessments and develop individualized care plans.
• Provide restorative, maintenance, and preventative therapy services.
• Evaluate home safety needs and coordinate home modifications and adaptive equipment.
• Train participants, caregivers, families, and staff on safe performance of daily living activities.
• Document assessments, treatment plans, progress notes, and outcomes in accordance with regulatory requirements.
• Monitor and maintain durable medical equipment inventory and utilization.
• Collaborate with the interdisciplinary team to achieve participant-centered goals.
• Supervise and provide guidance, competency assessments, and performance input for Certified Occupational Therapy Assistants (COTAs).
• Support quality improvement initiatives and uphold organizational values, compliance standards, and participant-centered care.

In this role, you will provide comprehensive occupational therapy evaluations, treatment, and care planning for older adults in the day center, nursing home, and home-based settings. You will work collaboratively with an interdisciplinary team to help participants maintain independence, safety, and quality of life.

• Minimum one (1) year of experience working with frail or elderly populations preferred; relevant clinical placement experience may be considered.
• Two (2) years of occupational therapy experience preferred.
• Strong communication, collaboration, and clinical decision-making skills.
• Ability to work effectively in home, community, and clinical settings.
