Job summary

Element Care is seeking a Registered Occupational Therapist to join our interdisciplinary team in Massachusetts. The role focuses on planning, implementing, and evaluating care plans within the PACE model of care.

The OT will collaborate with Physical Therapy and rehab teams, supervise COTA’s and rehab aides, conduct evaluations, and participate in care planning, family meetings, and discharge planning. This full-time position runs M-F 8am–4pm with benefits.

Qualifications

• Massachusetts license as an OTR/L.
• Graduate of an approved OT curriculum.
• Current CPR certification.
• 1–2 years of geriatric experience.

Responsibilities

• Plan, implement, and evaluate care plans per PACE model.
• Coordinate Rehab services; supervise COTAs and rehab aides.
• Document per PACE protocol and enter timely data.
• Conduct initial evaluations and develop treatment plans.
• Provide home safety evaluations and family education.
• Attend family and staff meetings and discharge planning.
