Job summary

Valir PACE in Shawnee, OK is seeking an Occupational Therapist (OT) to evaluate cognitive, physical, psychosocial, and perceptual functioning, promoting independence in meaningful daily activities. The OT develops and implements individualized treatment plans designed to enhance functional performance, improve quality of life, and support independence within the PACE model of care.

The OT leads evaluations, plans interventions, documents progress, and collaborates with an interdisciplinary team

Responsibilities

• Plan, manage, and coordinate participant evaluations and treatment interventions based on age-specific and individualized needs.
• Evaluate the whole participant by incorporating medical record data and assessing functional status, cognitive abilities, ADLs, IADLs, upper extremity function, coordination, sensory processing, and rehabilitation potential.
• Correlate evaluation findings to develop individualized treatment plans directed toward short- and long-term functional goals.
• Provide therapeutic interventions focused on improving functional performance, self-care, adaptive skills, fine motor coordination, upper extremity function, cognitive functioning, environmental access, and safety awareness.
• Analyze tasks and adapt environments, tools, equipment, and treatment approaches to meet participant needs and sociocultural context.
• Recommend, train, and educate participants, caregivers, and interdisciplinary team members regarding adaptive equipment, compensatory techniques, and home safety modifications.
• Design individualized home programs and provide instruction to participants, families, and ancillary personnel regarding treatment strategies and equipment use.
• Monitor participant response to treatment and modify the plan of care as clinically indicated in collaboration with the interdisciplinary team.
• Participate in interdisciplinary care conferences and collaborate with team members to ensure continuity of participant-centered care.
• Complete timely and accurate documentation in accordance with established policies, regulatory standards, and professional practice guidelines.
• Schedule treatments according to participant needs, caseload demands, and coordination with interdisciplinary services.
• Delegate appropriate interventions to therapy support personnel according to scope of practice while maintaining responsibility for participant outcomes and treatment programs.
• Utilize supplies and equipment in a cost-effective manner and complete appropriate charge capture procedures.
• Maintain compliance with all organizational policies, professional standards, and regulatory requirements.
• Perform all other duties as assigned.

Qualifications: Job Summary:
