Job summary

Providence PACE in Beaverton, Oregon, is seeking a full-time Occupational Therapist to support our Tanasbourne center and Washington County catchment. You will perform OT evaluations and develop and implement the OT plan of care within PACE policies as part of the Interdisciplinary Team.

You will collaborate with other providers, caregivers and family members to coordinate care, helping older adults remain active and independent in the community.

Qualifications

  • Bachelor's Degree in Occupational Therapy
  • Oregon Occupational Therapist License
  • National Provider BLS - American Heart Association within 30 days of hire

Responsibilities

  • Perform OT evaluations and develop and implement the OT plan of care per PACE policies.
  • Communicate and coordinate care with the Interdisciplinary Team and outside providers.
  • Coordinate with caregivers and family members to support community-dwelling elders.

This is a full time position supporting our Tanasbourne center and Washington County catchment.
This position is responsible for OT evaluations, and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members.

Required Qualifications
  • Bachelor's Degree in Occupational Therapy
  • Upon hire: Oregon Occupational Therapist License
  • Within 30 days of hire: National Provider BLS - American Heart Association
Preferred Qualifications
  • Master's Degree in Occupational Therapy
  • OTD (Occupational Therapy Doctorage)
  • 1 year of experience working as an occupational therapist in a geriatric setting.
