Job summary

Providence Health Plan Group is seeking an Occupational Therapist to support our Tanasbourne center and the Washington County catchment. This full-time role performs OT evaluations and develops plans of care according to PACE policies, joining an Interdisciplinary Team.

The OT communicates with the team and coordinates care with outside providers, caregivers and family members to empower elders to live active, independent lives within the community.

Qualifications

• Bachelor's Degree in OT required.
• Oregon OT license required upon hire.
• BLS certification (American Heart Association) within 30 days of hire.

Responsibilities

• Perform OT evaluations.
• Develop and implement OT plan of care per PACE policies.
• Communicate and coordinate care with the Interdisciplinary Team and outside providers, caregivers and family members.

This is a full time position supporting our Tanasbourne center and Washington County catchment.

This position is responsible for OT evaluations, and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members.

Required qualifications:

• Bachelor's Degree in Occupational Therapy
• Upon hire: Oregon Occupational Therapist License
• Within 30 days of hire: National Provider BLS - American Heart Association

Preferred qualifications:

• Master's Degree in Occupational Therapy
• OTD (Occupational Therapy Doctorage)
• 1 year of experience working as an occupational therapist in a geriatric setting.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
