Providence PACE in Beaverton, Oregon is seeking a dedicated Occupational Therapist to support the Tanasbourne center and Washington County. You will evaluate residents, develop and implement the OT plan of care, and coordinate with the Interdisciplinary Team and families to ensure seamless, person-centered care.

Responsibilities

  • Perform OT evaluations and develop the OT plan of care per PACE policies.
  • Implement and coordinate the OT plan of care with the Interdisciplinary Team.
  • Communicate and coordinate care with outside providers, caregivers and family members.
  • Collaborate to empower elders to live independently within the community.

Job description

Description

This is a full time position supporting our Tanasbourne center and Washington County catchment.

This position is responsible for OT evaluations, and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members.

Required qualifications:
  • Bachelor's Degree in Occupational Therapy
  • Upon hire: Oregon Occupational Therapist License
  • Within 30 days of hire: National Provider BLS - American Heart Association
Preferred qualifications:
  • Master's Degree in Occupational Therapy
  • OTD (Occupational Therapy Doctorage)
  • 1 year of experience working as an occupational therapist in a geriatric setting.
