Providence PACE seeks a dedicated Occupational Therapist to support our Tanasbourne center and Washington County catchment. You will perform OT evaluations and develop an OT care plan aligned with PACE policies, while coordinating with the Interdisciplinary Team, providers, caregivers, and families to optimize patient outcomes.
The role emphasizes geriatric care, community-based support, and collaboration to empower elders to live active, independent lives within the Providence network.
This is a full time position supporting our Tanasbourne center and Washington County catchment.
This position is responsible for OT evaluations, and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members.
