Job summary

Simpsonville Post Acute is seeking a compassionate Occupational Therapist (OTR) to deliver patient-centered therapy to residents, focusing on evaluation, treatment planning, and collaboration with the care team.

Qualifications

• Degree in Occupational Therapy (Bachelor's required; Master's or Doctorate preferred).
• Active SC Occupational Therapist license (in good standing).
• Experience in skilled nursing or rehab setting preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapy services per physician orders and care goals.
• Document evaluations, treatments, and progress notes accurately and timely.
• Communicate with physicians, nursing staff, and interdisciplinary team members.
• Educate residents, families, and caregivers on therapy programs.
• Participate in care planning and discharge planning.
• Conduct staff in-services and training.
• Maintain compliance with all state, federal, and payer regulations.

Position Summary

We are seeking a compassionate and skilled Occupational Therapist (OTR) to provide patient-centered therapy services that improve residents' independence and quality of life. This role focuses on evaluation, treatment planning, and collaboration with the interdisciplinary care team.

Key Responsibilities

• Evaluate residents and develop individualized treatment plans
