Job summary

Brushy Creek Post Acute in Greer, SC is seeking a compassionate Occupational Therapist (OTR) to join its rehabilitation team on a PRN basis. You will evaluate residents, design individualized plans, and work with families and physicians to maximize independence and quality of life.

You will provide skilled therapy per orders, monitor progress, educate families, and participate in care planning while maintaining accurate documentation and compliance with state rules.

Qualifications

• Bachelor's degree in Occupational Therapy required.
• Master's or Doctorate degree in Occupational Therapy preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with PCC and Casamba preferred.
• Strong clinical, communication, and documentation skills.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services per physician orders and resident goals.
• Monitor resident progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, residents, families, and interdisciplinary teams.
• Participate in care plan meetings, rehabilitation conferences, and discharge planning.
• Complete accurate documentation in accordance with facility and regulatory requirements.
• Educate residents, families, and caregivers on treatment plans and maintenance programs.
• Recommend adaptive equipment to improve function and safety.
• Conduct in-service training for facility staff as needed.
• Maintain compliance with facility policies and state practice acts.
• Promote resident safety, independence, and well-being.

Occupational Therapist (OTR) - PRN

Position Summary

We are seeking a compassionate and skilled Occupational Therapist (OTR) to join our rehabilitation team on a PRN basis. • Monitor resident progress and adjust treatment plans as needed
• Collaborate with physicians, nursing staff, residents, families, and interdisciplinary team members
• Participate in care plan meetings, rehabilitation conferences, and discharge planning activities
• Complete accurate and timely documentation in accordance with facility, state, federal, and payer requirements
• Educate residents, families, and caregivers on treatment plans, adaptive techniques, and maintenance programs
• Recommend adaptive equipment and interventions to improve resident function and safety
• Conduct in-service training and education for facility staff as needed
• Maintain compliance with all facility policies, state practice acts, and regulatory requirements
• Promote resident safety, independence, and overall well-being

Qualifications

Education & Experience

• Bachelor's degree in Occupational Therapy required
• Master's or Doctorate degree in Occupational Therapy preferred
• Experience in skilled nursing, rehabilitation, or long-term care preferred
• Experience with PointClickCare (PCC) and Casamba preferred
• Strong clinical, communication, and documentation skills

Licenses & Certifications

• Current Occupational Therapist license in South Carolina and in good standing
• Must maintain all required continuing education requirements
• Must remain in good standing with the applicable State Board
