Job summary

Simpsonville Post Acute is seeking a skilled Occupational Therapist (OTR) to provide patient-centered therapy on a PRN basis at our Simpsonville, SC facility. You will evaluate residents, develop individual treatment plans, and deliver therapy aligned with physician orders and care goals.

The role requires SC licensure (OTR) and a background in skilled nursing or rehab settings; familiarity with PCC and Casamba is preferred to support interdisciplinary care and accurate documentation.

Qualifications

• Bachelor’s degree in Occupational Therapy (Master’s/Doctorate preferred).
• Active South Carolina OT license (OTR) in good standing.
• Experience in skilled nursing or rehab setting preferred.
• Familiarity with PCC and Casamba preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapy services per physician orders and care goals.
• Document evaluations, treatments, and progress notes accurately and timely.
• Collaborate with physicians, nursing, and therapy team members.
• Educate residents, families, and caregivers on therapy programs.
• Participate in care planning and discharge planning.
• Conduct in-services and staff training as needed.
• Ensure compliance with state, federal, and payer guidelines.
