• Evaluate residents and develop individualized treatment plans.
• Provide therapy services per physician orders and care goals.
• Document evaluations, treatments, and progress notes accurately and timely.
• Collaborate with physicians, nursing, and therapy team members.
• Educate residents, families, and caregivers on therapy programs.
• Participate in care planning and discharge planning.
• Conduct in-services and staff training as needed.
• Ensure compliance with state, federal, and payer guidelines.
