  • Evaluate residents and develop individualized treatment plans that promote independence and functional recovery
  • Provide evidence-based interventions focused on ADLs, mobility, safety, and quality of life
  • Supervise and mentor COTAs
  • Collaborate with physicians, nursing staff, and rehabilitation professionals to achieve optimal outcomes
  • Participate in care planning meetings, utilization reviews, and discharge planning discussions
  • Maintain accurate, timely, and compliant documentation
  • Educate residents, caregivers, and family members on treatment strategies, home safety, and adaptive equipment recommendations
  • Support successful transitions home through comprehensive caregiver training and discharge planning

Key Responsibilities

  • Participate in care planning meetings, utilization reviews, and discharge planning discussions
  • Maintain accurate, timely, and compliant documentation, including evaluations, progress notes, and discharge summaries
  • Educate residents, caregivers, and family members on treatment strategies, home safety, and adaptive equipment recommendations
  • Support successful transitions home through comprehensive caregiver training and discharge planning
