Job summary

Pleasant Hill Post Acute in Pleasant Hill, CA is seeking a compassionate Occupational Therapist to join our rehabilitation team. We offer Full-Time, Part-Time, and PRN opportunities for OTs who want to help patients regain independence and achieve the highest level of function.

As part of our interdisciplinary team, you will evaluate patients, develop individualized plans, provide skilled OT services, and document progress.

Qualifications

• Current California OT license in good standing.
• Strong clinical, communication, and organizational skills.
• Ability to work effectively in an interdisciplinary team.
• New graduates are welcome to apply.
• Post-acute rehabilitation experience preferred.

• Evaluate patients and develop individualized treatment plans based on their functional needs and goals.
• Provide skilled occupational therapy services to help patients improve independence with activities of daily living.
• Help patients maximize functional abilities and improve quality of life.
• Monitor progress and adjust treatment plans as appropriate.
• Collaborate with physicians, nursing staff, physical therapists, speech therapists, and other team members.
• Communicate with patients, families, and caregivers regarding goals and progress.
• Complete accurate, timely clinical documentation.
• Provide compassionate, patient-centered care while maintaining professional standards.

Our rehabilitation program emphasizes quality patient care, teamwork, communication, and collaboration across nursing, therapy, and other departments.

What You’ll Do
