Job summary

Centra Healthcare Solutions is seeking an Occupational Therapist for a skilled nursing facility in Sacramento, CA. We are looking for an experienced professional or a new graduate for a direct hire/permanent role.

You will assess referred patients, develop treatment plans, set goals, document progress, and educate patients and families on OT methods, while collaborating with the care team to achieve rehab objectives.

Qualifications

• Graduated from an accredited OT program.
• Current state OT license or actively pursuing licensure in the state.
• Current CPR certification is required.

A great Skilled Nursing Facility (SNF) is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a direct hire / 'perm' assignment!

Responsibilities

• Assess referred patients requiring rehabilitation services
• Provide direct patient care to assess medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation
• Establish and administer a treatment program with specific goals based on the patient’s capacity and tolerance under physician direction
• Set realistic and achievable goals for patients, document and record patient condition, and educate patients and families on occupational therapy methods
• Provide skilled occupational therapy services/interventions in accordance with physician orders
• Assess patient needs, plan for, evaluate, and modify care to meet goals of occupational therapy interventions
• Collaborate with all disciplines to plan and evaluate team goals for each patient

Qualifications

• Graduated from an accredited school
• Current state license as an Occupational Therapist / OT or actively in the process of obtaining current state licensure
• Current CPR certification
