Job summary

Evolve Therapy Services is hiring compassionate Occupational Therapists for an in-house Skilled Nursing Facility partner in Altoona, PA. The OT will plan, organize, develop and direct therapy services, supervise COTA as needed, and ensure documentation and billing align with regulations and quality standards. This PRN role emphasizes collaboration with an interdisciplinary team to maximize resident outcomes and care quality.

Qualifications

• Degree in Occupational Therapy
• Active OT license in practicing state(s)
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Conduct regular screenings of residents to identify best intervention or treatment.
• Evaluate residents to inform treatment plans; establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules in coordination with team members.
• Deliver therapy per treatment plan and ensure proper documentation and billing.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
